Substance Use / Chemical Dependency Evaluations

I perform Substance Use Evaluations (also known as Chemical Dependency Evaluations) for adolescents and for adults.  I am a Registered Service Provider with the criminal justice system in the State of Nebraska.  I have been trained in and use the Addiction Severity Index (ASI) for adults and the Comprehensive Adolescent Severity Inventory (CASI) for adolescents.  Please see above "Substance Use / Chemical Dependency Evaluation Fee" for information on the fee and method of payment for evaluations.

If you are in need of a Substance Use Evaluation, the process is as follows:

* Please arrive in person at the Great Oaks Counseling office, 13906 Gold Circle, Suite 202.  The receptionist is available to take your payment (cash only) and provide you with the paperwork Monday through Thursday (with the exception of observed holidays), 8:30 AM to 4:00 PM.  Please allow 20 minutes to complete this initial paperwork.  You may wish to call the receptionist ahead of time to be sure she is in the office, 402-932-6500 extension 100.

* Once the $200 fee is paid and the initial paperwork completed, I will call you to schedule your Substance Use Evaluation appointment.

I am an EMDR (Eye Movement Desensitization and Reprocessing) Certified Therapist.  EMDR is a very effective therapy for processing traumatic events, painful experiences and memories, recurrent triggers, fears, and areas where a person feels "stuck," including with grief and loss.  It is also effective with depression and anxiety.

I was trained by EMDR Institute senior trainers Dr. Gerald Puk and Dr. Curt Rouanzoin.  I am a member of the EMDR International Association (EMDRIA).

Are you having trouble moving forward in your life?  Have you had losses that you are having difficulty healing from?  Consider EMDR therapy for yourself for healing and wholeness.
